- Whether or not this person can read

- Whether or not this person can write

- Name and address of closest relative in country of origin

- Has person ever been in the US before
- Name of person and address of person the border crosser intends to visit
- Description of person crossing the border including health, deformities, height, complexion, hair, eyes, marks of identification

- Name of ship
